Foundayo (orforglipron) price: $649 list, $149 to $349 self-pay, and the $50 Medicare Bridge

Foundayo is orforglipron, a small-molecule oral GLP-1 receptor agonist taken once daily, approved by the FDA on April 1, 2026 for adults with obesity, or overweight with a weight-related condition, alongside diet and exercise. It is the first GLP-1 pill for weight management that is not a peptide, and it launched with a price structure set out in the November 2025 government agreement five months before approval.

List price

$649 per 30-day supply at every strength, per Lilly's Colorado wholesale acquisition cost disclosure sheet with a data date of April 2, 2026. That is the lowest list price in this index, about 60 percent of Zepbound's and under half of Wegovy's.

Self-pay through LillyDirect

Lilly's coverage page, read September 4, 2026, states regular one-month (30-day) prices by strength and a purchase offer at the two highest strengths:

The $299 offer at 14.5 and 17.2 mg applies on the first purchase and on refills bought within 45 days of the previous delivery; miss the window and the $349 regular price applies until you re-qualify. Lilly defines a Foundayo month as 30 days, so unlike the 28-day injectable fills, a per-fill price here is close to a calendar-month price: $299 per 30 days is $3,640 a year, $303 per month.

Savings card

Lilly's approval release says eligible commercially insured patients may pay as little as $25 per month with the Foundayo savings card, terms and conditions applying. The per-fill and annual caps were not readable as text on the access date; the calculator asks you to enter them from your card rather than assuming Zepbound's.

Medicare

Foundayo is one of the three products in the Medicare GLP-1 Bridge: $50 per month for eligible Part D enrollees from July 1, 2026 to December 31, 2027, under CMS clinical criteria. Lilly's release put it plainly: eligible Medicare Part D individuals may be able to get Foundayo for $50 per month beginning as soon as July 1, 2026. See the Medicare page.

Reading the ladder

Two things stand out against the injectables. The starting strength is priced at $149, the same figure Novo Nordisk uses for the Wegovy and Ozempic pills at 1.5 mg, which is the number the November 2025 agreement fixed for starting doses of oral GLP-1s. And the top of the ladder is $299 to $349 rather than $449, which narrows the gap between a maintenance dose and a starting dose. Per milligram the comparison means little: orforglipron is a different molecule with its own potency, so a $ per mg figure across Foundayo and tirzepatide compares nothing.
