About the GLP-1 Pricing Index
What this site records, who runs it, how prices are collected and dated, and how to report a figure that has changed.
GLP-1 Pricing Index is the price desk of the FormBlends network. It records what GLP-1 medication costs in the United States, where each figure came from, and on what date it was read.
What the site is
Three kinds of page. Price pages record brand list prices, manufacturer self-pay tiers and savings-card terms for Zepbound, Mounjaro, Wegovy, Ozempic, the Ozempic pill that replaced Rybelsus, Saxenda and Foundayo. Coverage pages record the Medicare, Medicaid and employer position, with the statute, the CMS program terms and the IFEBP survey numbers. Methodology pages explain how compounded offers were collected, what all-in means, why price per milligram is arithmetic and not a quality signal, and why a compounded vial and a brand pen are not prices for the same thing. One calculator turns any of those prices into a real monthly, per-dose and per-milligram figure and shows every step.
Who runs it
The FormBlends editorial team, which also publishes formblends.com. FormBlends operates a GLP-1 weight-loss program that dispenses compounded semaglutide and tirzepatide through licensed pharmacies. That makes FormBlends a competitor of every compounded provider in the index and a seller of a product this site prices. The interest is disclosed here, in the methodology, and on every page that touches compounded pricing. FormBlends' own offer sits in the compounded table under the same rules as everyone else's and is never highlighted.
How prices are collected
- Brand and program prices are read from the manufacturer's own page (Lilly, Novo Nordisk, NovoCare, LillyDirect), the relevant government page (CMS, TrumpRx.gov, the Social Security Act) or a dated press release. The page URL and access date sit under every table. Where a manufacturer page renders a figure only in a browser session and could not be captured, the page says so and cites the next best dated source.
- Compounded offers come from the weekly Compounded GLP-1 Price Index compiled by the FormBlends research team, republished here as data with its collection date. Prices read from the provider's own public page are marked direct; prices captured by hand because a site forbids automated reading are marked manual; prices that exist only behind a quiz and were taken from a named third-party report are marked secondary and never enter a median.
- Every number on a page cites its source inline or in the sources list at the foot of the page. A number we could not source is not published.
- Pages carry an updated date. When a price, a card term or a program changes, the page changes and the date moves.
Editorial rules
No reviews, testimonials, star ratings or named clinicians. No AggregateRating schema. No claim that a page was medically reviewed when it was not. Compounded drugs are described as what they are: not FDA approved, not interchangeable with brand products, dispensed under a prescription. No provider is ranked or recommended. The calculator never pre-fills a dose; it offers published prices as presets you can click, and it leaves the strength to your prescription.

Prices on this subject move often. If a figure here no longer matches its source, or a program has changed terms, write to hello@formblends.com with the page URL, the figure, and the source page you are looking at. Corrections are made as a priority and noted on the page.
