TrumpRx and the November 2025 pricing agreement: what changed for GLP-1 prices

On November 6, 2025 Lilly and Novo Nordisk each announced an agreement with the U.S. government covering the price of their GLP-1 medicines to Medicare, to Medicaid, and to cash-paying patients. TrumpRx.gov, the government's directory of those cash prices, went live on February 6, 2026. The agreements are why almost every price on this site moved in the winter of 2025 to 2026.

What the agreements set

Per the AMCP summary of the announcement and the companies' releases:

  • Injectable Ozempic, Mounjaro, Wegovy and Zepbound priced at $245 per month to Medicare, for all doses and indications, with the same price available to state Medicaid programs.
  • A $50 monthly copay for Medicare beneficiaries prescribed an anti-obesity medication, delivered through what became the Medicare GLP-1 Bridge on July 1, 2026 (Lilly's release had said as early as April 1, 2026).
  • Direct-to-consumer cash prices averaging $350 per month for vials, described as a 30 percent discount from the $499 self-pay prices of the time, with starting doses of the oral products at $150 per month.
  • Lilly: the Zepbound multi-dose pen at $299 for the lowest strength and up to $449 for the others, a $50 cut from then-current LillyDirect prices; orforglipron (Foundayo) from $149 to $399 on approval.
  • In return, the companies received three years of tariff relief and a commitment against future pricing mandates.

Novo Nordisk implemented its side on November 17, 2025 with the $349 ongoing price and the $199 two-fill introduction. Lilly cut vials to $299 to $449 on December 1, 2025.

Each TrumpRx page carries the note that the figure is an out-of-pocket price and that an insured person should check their copay first. The White House release put the launch-day figures as Ozempic from $1,028 to an average of $350, injectable Wegovy from $1,349 to an average of $350 and as low as $199, and Zepbound from $1,088 to an average of $346 and as low as $299.

What TrumpRx is and is not

It is a directory. The prices are the manufacturers' own self-pay prices, and the buy button sends you to LillyDirect or NovoCare's channel. It does not take insurance; the release calls for legislation to let health plans count TrumpRx purchases, which as of the access date has not happened, so a TrumpRx purchase does not run through your deductible. It does not list compounded products.

The trend-down clause

The agreement described the $350 average cash price as trending down to $245 over two years. As of September 4, 2026 the published self-pay prices are $299 to $449 for Zepbound, $349 for the Wegovy and Ozempic pens and $499 for Mounjaro; the $245 figure is the Medicare and Medicaid price, not yet a cash price anywhere. This page will record the date if a manufacturer publishes a lower cash tier.
