Mounjaro price: list, the $499 self-pay figure, and the savings card caps

Mounjaro is tirzepatide approved for type 2 diabetes. Same molecule and the same six strengths as Zepbound, a separate product with a separate list price, a separate card, and a different route through Medicare.

List price

TrumpRx.gov shows the Mounjaro original price as $1,112.16 for a 28-day fill of four single-dose pens (read September 4, 2026). Lilly's own Colorado wholesale acquisition cost disclosure sheet, the only Lilly document that could be captured as text on the access date, lists $1,079.77 per fill for every strength with a data date of January 2, 2025. Both are recorded so the year-on-year increase is visible; the 2026 figure is the one to use.

Self-pay

TrumpRx.gov prices Mounjaro at 55 percent off the original figure, a saving of $613.16, which is $499 per 28-day fill at every strength. Lilly's own savings page reaches the same number by another route: commercially insured patients whose plan does not cover Mounjaro can use the card to pay as low as $499 for a one-month fill, with the card contributing up to $647 per fill and $8,411 per year. There is no strength ladder; 2.5 mg and 15 mg cost the same per fill, which makes the per-milligram figure fall sharply with the dose (see price per mg explained).

Savings card with commercial coverage

For a commercial plan that covers Mounjaro: as little as $25 for a 1-, 2- or 3-month fill; the card pays up to $150 per one-month fill, $300 per two-month, $450 per three-month; the annual cap is $1,950 across a maximum of 13 fills per calendar year; the card expires December 31, 2026. Month is defined as 28 days and up to four single-dose pens. Government beneficiaries are excluded, as are Massachusetts residents where a generic equivalent exists and California residents where an FDA-approved therapeutic equivalent exists. Because the annual cap is $1,950 and the per-fill cap is $150, the cap binds at exactly 13 fills; a plan copay of $175 leaves you at $25 for all 13, while a copay of $250 leaves you at $100 per fill because the per-fill cap is hit first. Medicare and Medicaid

Mounjaro is not part of the Medicare GLP-1 Bridge, which is for weight-management products. Prescribed for type 2 diabetes it is an ordinary Part D drug subject to the plan's formulary and tier. Under the November 6, 2025 agreement between the manufacturers and the U.S. government, injectable Ozempic, Mounjaro, Wegovy and Zepbound are priced at $245 per month to Medicare and to state Medicaid programs for all doses and indications, per the AMCP summary of the deal; that is the price the program pays, not a beneficiary copay. Mounjaro versus Zepbound on price

A prescriber picks the product for the diagnosis. Using Mounjaro for weight loss is off-label, and the Mounjaro card terms require it to be prescribed for its approved use. The figures are here so a person with a diabetes prescription can see their own numbers, not so anyone can shop between the two.
