A list price is the wholesale acquisition cost, the number a manufacturer publishes as the price to wholesalers before any rebate, discount, insurance or self-pay program. It is the anchor every other price is measured from, so it is the first thing this index records. Each row carries the unit the price is per, the page it was read from, and the date.

Two notes on the Lilly rows. Lilly's pricing page for Zepbound (pricinginfo.lilly.com/zepbound) draws the figure in the browser and could not be captured as text on the access date, so the Zepbound row cites the February 6, 2026 government release, which quotes the monthly figure rounded to the dollar. Mounjaro has two figures because the Lilly disclosure sheet available on the access date carries a January 2025 data date while the TrumpRx page shows the current 2026 figure; both are given so the increase is visible.
What is changing
Novo Nordisk announced on February 24, 2026 that the U.S. list price of Wegovy, Ozempic and Rybelsus will be $675 per month from January 1, 2027, a cut of about 50 percent for Wegovy and 35 percent for Ozempic. The release states that self-pay prices are not affected. Rybelsus has since been retired as a U.S. brand and continues as the Ozempic pill, so that row inherits the change.
Separately, CMS announced negotiated Medicare prices for 2027 in November 2025; the semaglutide products (Ozempic, Wegovy, Rybelsus) carry a negotiated price of $274 per month from January 2027. That is a Medicare price, not a list price, and it is covered on the Medicare page.
Why the list price still matters
- Deductibles. If your plan makes you pay the full cost until a deductible is met, the full cost is the plan's negotiated price, which tracks the list price.
- Coinsurance. A 25 percent coinsurance on $1,349.02 is $337; on $675 it is $169. The 2027 change will show up in coinsurance long before it shows up anywhere else.
- Self-pay programs are described as discounts from list. TrumpRx.gov shows each price as a percentage off the original figure in the table above.
- Compounded prices have no list price at all. A compounding pharmacy sets its own price and it is not a discount from anything, which is one reason the comparison page exists.

Does anyone actually pay the list price?
Very few people. The list price is the wholesale acquisition cost before rebates, insurance, savings cards or self-pay programs. It matters because deductibles, coinsurance and some out-of-network claims are calculated from it, and because the self-pay programs are priced as a discount from it.
Why does the same drug have a different list price as Zepbound and Mounjaro?
They are separate products with separate approvals (chronic weight management and obstructive sleep apnea for Zepbound, type 2 diabetes for Mounjaro) and each has its own list price. The molecule and the strengths are the same.
